At Alabama Orthopaedic Center, P.C., we understand that each patient is unique and we tailor an individual treatment regimen for each with the ultimate goal being able to return to an active and productive life style.

In order for us to provide the best care for you, it is very important that you bring the following items to your visit:

A list of your current medications
Any and all x-ray or MRI films
Insurance information and card
Information about what treatments and medications have been tried in the past
Medical records from prior visits that you think may be helpful

New Patient Appointment Scheduling

• To make an appointment please call our main number (205) 802-6700. 

• Have your insurance card available so that we can help you obtain the optimum benefit.
  
  

• We will make every effort to schedule your appointmentfor your convenience.  If your injury requires same or next day treatment, we will do our best to work you in.
  
  

• To save time, you may print and complete the required new patient forms on-line prior to your appointment.  If not, please arrive 20 minutes prior to your scheduled appointment time to complete the required forms in our office.
  
  

• If your insurance requires a referral from your primary care physician, please contact your doctor to request the referral.  Many offices require 24-48 hours to process the referral and forward it to us.
  
  

• Please have your insurance card and driver's license available.
  
  

• If you have x-rays, MRIs or other imaging studies related to your injury, please bring them to your appointment.  This will allow us to avoid duplicating the service and related cost.
  
  

• We will collect co-pays, as required by your insurance plan, at the time of your appointment.  We accept cash, checks and Visa or Mastercard.
