Credit Policy
The following credit policy has been established in order to preserve our equitable fees.
For Physician Professional Fees
If the patient does not have insurance or insurance does not cover the procedure, payment arrangements must be made with the billing department prior to the date of surgery.
For Office visits
- If the patient does not have insurance, a minimum payment of 100.00 will be required at the time of service.
- All co-pays are due at the time of service. We will bill the patient for any balances that the carrier indicates are “patient responsibility”. Payment for these balances is due within thirty days. If payment arrangements need to be made, the patient can contact the billing department at 205.802.6779.
For Surgeries
Each patient’s insurance carrier will be contacted to verify coverage, pre-admission requirements, deductibles and co-pays.
